Enquiry for Documents required along with internship certificate

Hello sir
I am currently completing the internship project based on web development.
After completing the internship, my college want some other documents as well along with internship certificate that are-
1)Attendance sheet
2) Abstract performa
3) Evaluation Performa.

Kindly let me know how can I get these documents after i complete my internship.
